Now you can order the complete Siamese Plate On the Go menu using a handy FAX order form. Use FAX ordering for pickup at one of the restaurants or for delivery to your home or business
in the Boulder, Colorado area!
Please read the instructions below before you proceed with your order.

1. There is a separate FAX order form for each restaurant. You must order using the correct form.  Click the links above to order from the Boulder 28th Street and Boulder Folsom Streetrestaurants. If you use the incorrect form, your order will go the wrong place and have the wrong items and prices!
2. Select Thai & Japanese Appetizers. 
3. Fill out Soup & Salad, Sushi, Entree orders for each person using the forms for Orders 1-4; (Note that Thai Soup serves 2 people).
4. Specify ingredient options, if available, for the items.
5. Fill out personal information at the bottom of the page, including required name and phone number to verify order.
6. Specify the time you want to pick up your order at the Siamese Plate or have it delivered in Boulder, Colorado. The delivery charge is $4.00 per destination.
7. Check your order carefully to make sure it is correct.
8. Press the "Create FAX Order" button at bottom of the order page ONCE per group of up to 4 orders.
9. Review your FAX Order on the FAX Ordering page. 
10. Please print out the FAX order form and FAX it to the number provided on the form!
11. To order for more than four people, create and FAX another form (allow 10 additional minutes per form.)
12. You should receive a confirmation call from the restaurant within 15-20 minutes. If you don't, please phone the restaurant at the number provided on the FAX form.
13. Pick up your order at the restaurant (see locations below) or wait for the delivery person to arrive.
14. Please allow at least 30 minutes for take-out and 60 minutes for delivery within hours: 11:00 am - 2:00 pm & 5:00 pm - 10:00 pm, DAILY.
15. Cancellations must be phoned to the proper restaurant within 15 minutes of submittal.
16. If applicable, any special discounts will be credited to you when you pick up or receive your order.
